United Health Centers of the San Joaquin Valley (UHC) is a comprehensive healthcare system that focuses on helping medically underserved residents of the San Joaquin Valley access affordable healthcare, wellness programs, and community resources. To further this mission, UHC is developing its first PACE (Program of All-Inclusive Care for the Elderly) Center. The PACE model integrates medical, behavioral, therapeutic, and social services, including primary care, rehabilitation, transportation, meals, and in-home assistance.
UHC's 24,368 sq.ft facility features primary care clinic space, a commercial kitchen, therapy spaces, personal care areas, and an interdisciplinary care team coordination center. The facility will provide comprehensive, coordinated care to seniors who require nursing home-level care but wish to remain safely in their homes and communities. Services will be delivered 24/7, 365 days a year, with on-site support coordinated closely with UHC’s extended network of hospitals, specialists, and in-home care providers.
